Credentials

Player Passes – Must have 2024/2025 USCS/CYSA laminated passes

Medical Release Forms – 1601 for CalNorth (usually printed on the back of the card) or form #R002-Youth: Youth Player Registration & Medical Treatment Authorization Form – for USCS

Official Roster – USCS or CYSA Golden Rod

US Club Guest Player Form

Cal North Guest Player Form

Withdrawal penalties

$100 if withdrawal is requested in writing prior to the registration deadline.  If the request is made after the registration deadline or after the team has been accepted the whole registration fee is forfeited.

Late Registration penalties

$75 after the deadline posted above
