Waiver and Release: In consideration of being permitted to participate in any way, including but not limited to trainings, try outs, practices, playing, coaching, or being transported to or from games or events related to or with Elk Grove Soccer, I, for myself, my heirs, my estate, my executor, my personal representatives or assigns, and all members of my family, do hereby release, waive, discharge, and agree not to sue Cosumnes Community Services District (CSD), and Elk Grove Soccer, its Board of Trustees, officers, employees, agents, and volunteers and will not hold them liability for any and all claims resulting in personal injury, accident or illness (including death), and property loss arising from, but not limited to, participation in any Elk Grove Soccer Activity.
SUPPLEMENTAL WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT.

In consideration of being allowed to participate in sports and athletic activities with Elk Grove Soccer, as well as all related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as it pertains to protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such circumstance to the attention of the nearest official immediately; and,:

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Cosumnes Community Services District (CSD), and Elk Grove Soccer, their officers, officials, coaches and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law; and,

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND AM SIGNING IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Acknowledgement of Understanding: I have read this waiver, release of liability, assumption of risk, and agreement to indemnify and hold harmless, fully understand its terms, and understand that I am giving up substantial rights, including my rights to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend for my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. I understand and intend for this waiver, release of liability and assumption of risk agreement to be binding not only to me but also my heirs, my estate, my executor, my personal representatives or assigns and all members of my family.
